with the following information:

- We will ensure that all placements are made in accordance with any relevant legislative requirements.

By agreeing to these terms and conditions, I hereby acknowledge receiving and reading a copy of this "Information Statement for Work Seekers".
- The candidate is required to honestly and in good faith provide the necessary information for Medical Connect Australia to place the candidate within his/her specifications. Medical Connect
Australia is not responsible for the candidate's capabilities, integrity and general suitability to the position.
- The candidate authorizes Medical Connect Australia to distribute information provided by, and/or about the candidate in the interest of securing employment on behalf of the candidate.
- The candidate shall agree to keep and render to the practice/hospital or facility with whom they are placed true and proper accounts of all professional attendances and all other business
transacted on behalf of the practice, hospital or facility.
- The candidate warrants that should the position offered require individual medical indemnity, the candidate shall maintain membership of a recognized medical indemnity organization at
all times.
- The candidate shall attend to patients and cases promptly and exercise all reasonable care and skill in any treatment given and prescribed at the times agreed.
- The candidate undertakes to ensure that he/she has checked actual remuneration, conditions and payment methods/intervals with the practice, hospital or facility or their representative prior
to commencement of duty.
- The candidate shall not cease a placement before the agreed time, nor change any part of the arrangements without consent of the practice, hospital or facility and Medical Connect Australia.
- The candidate shall not accept any changes to, or extensions of the candidate's assignment or offer of future engagements (locum or assistant) including referral to another facility, written
or verbal, by the employer during a rolling 12 month period as an employee, without the facility agreeing to proceed through Medical Connect Australia.
- The candidate shall notify Medical Connect Australia of any variation to the originally negotiated terms of employment during the term of the employment.
- The candidate acknowledges that the practice, hospital or facility will be asked to provide an evaluation of the candidate on his/her performance at each placement. This information shall
remain the property of Medical Connect Australia.

- Medical Connect Australia will provide written confirmation of placement with the practice/hospital or facility. An assignment or position is deemed confirmed upon Medical Connect Australia
written confirmation. Any changes notified to Medical Connect Australia, once confirmed, must be in writing.
- Medical Connect Australia cannot guarantee the placement if changes to the agreed arrangements are made without notifying Medical Connect Australia.
- Medical Connect Australia accepts no responsibility or liability for any loss, expense or damage arising from any failure by the candidate or the practice, hospital or facility, howsoever
arising.
Medical Connect Australia has the right to refuse to accept, or not to continue, any medical practitioner on the register as seeking placement. The decision of Medical Connect Australia in this
matter is final.
In the case of a dispute over Terms and Conditions of employment, the candidate shall attempt to resolve the matter directly with the practice, hospital or facility. If this is not successful
the candidate may use the services of a representative. If this is not successful, the candidate may apply to the Manager of Medical Connect Australia to facilitate and appoint a mediator/arbitrator
who is acceptable to both parties.
The candidate acknowledges that Medical Connect Australia will inject considerable effort into attempting to locate suitable employment on their behalf. As such, the candidate agrees that they will
not enter into any private arrangement or agreement with any hospital, practice or facility to which Medical Connect Australia introduced the candidate.
